this is Azabujuban Clinic.
Azabujuban Clinic is a private clinic that operates by appointment only. We provide psychiatry and psychosomatic medicine services. Our doctors have experience in neurodevelopmental outpatient care, psychiatry, and internal medicine in both Japan and the United States. Our doctors and psychologists will provide support for everything from consultations on children’s development to mental illnesses such as ADHD and ASD, depression, insomnia, anxiety disorders, and stage fright.
Our clinic strives to provide a style of treatment that minimizes stress for our patients.

At our clinic, everything can be done with just your smartphone.
We provide online consultations from the first visit, and deliver medication to your home the next day* if necessary.
We complete the four steps of “reserve → examine → pay → receive medication” online.
If you need medication, we will prescribe the medication you desire at the clinic.
If you do not need medication, we provide psychological therapy without medication.
We will issue documentation to those who require reasonable accommodation or take time off work or school. If you need counseling, we will connect you to a psychologist.

About the doctor
Consultations will be conducted by doctors with clinical experience in pediatric neurodevelopment, psychiatry, and internal medicine in both Japan and the United States. For those who wish to be examined in English, consultations with doctors and counseling with psychologists are available in English. The doctors’ listening skills, reliable diagnostic skills, and psychotherapy are the result of many years of training in hospitals in Japan and the United States.

Medical fees
Initial consultation fee
●Includes doctor’s consultation and prescription fee
●Approximately 30-50 minutes (including intake)
Weekdays ¥15,000 (tax included)
Saturdays ¥18,000 (tax included)
*First-time patients and those who have been in the hospital for more than 3 months will be considered first-time patients.
Re-examination fee
● Includes doctor’s consultation fee and prescription fee

※ Patients who were under the doctor’s care from the previous job will be treated as re-visits
- If this is your second or subsequent visit and it has been within 3 months since your first visit, it will generally be considered a re-visit, but if you are consulting about a different symptom or problem, it may be considered a first visit.
There is no difference in price between in-person and online appointments.
Your doctor will suggest 15- or 30-minute slots for follow-up appointments as necessary.
We provide elective medical treatment because insurance medical fees do not allow for sufficient time to provide appropriate medical care.
We appreciate your understanding.
Please note that elective medical treatment fees are not eligible for medical expense deductions, medical assistance for persons with disabilities, or medical assistance for persons on welfare.
Counseling fee by psychologist
Weekdays 50 minutes ¥12,000 (tax included)
Saturdays 50 minutes ¥13,800 (tax included)
Prescription fees and drug costs
There is no prescription fee.
Prescription medications are provided at the clinic.
Prescription medications for insomnia and ADHD: ￥3,300 (30 tablets)
A month’s supply of other medications is also about ￥3,300.

Wechsler Intelligence Scale (WAIS-IV)
In addition to IQ, strengths and weaknesses are evaluated based on four indicators: “verbal comprehension,” “perceptual reasoning,” “working memory,” and “processing speed.” It is the most widely used intelligence test in the world and is used to evaluate characteristics specific to developmental disorders.

ADHD Self-report Symptom Checklist (ASRS)
This is a simple self-assessment test. You can receive a score based on the three major symptoms (inattention, hyperactivity, and impulsivity).
Inspection costs
●WAIS-IV test: ¥33,000 (tax included)
(This is a detailed test used to diagnose ADHD and ASD. Also recommended for those who want to know their IQ.)

*Price includes explanation of results and report.
*Cancellation after booking is subject to the same terms of use as for medical consultations.
*Cancellation is free up to 6 days before the scheduled date of the test, but a 50% cancellation fee will be charged up to 3 days before.
If you cancel after 3 days before, you will be charged the full amount.
Inspection time required
●WAIS-IV: Takes 2-3 hours. This varies from person to person, but the cost does not change depending on the time.
●ADHD simple screening test: takes about 10 minutes (can be taken at home)
●ASD simple screening test: takes about 10 minutes (can be taken at home)
Inspection results report
●WAIS-IV test: We will give you a test report 3 weeks after the test and explain the results.
(WAIS-IV results can be used for MENSA membership applications)
●ADHD simple screening test: We will let you know within about a week
